我们建议您阅读我们的 Surrogate 要求 在您着手填写此申请之前。surrogacy的相关要求旨在确保您在情感、医疗和身体方面都适合这一流程。如果您的回答与资格要求不符，您会立即知晓。未能达到成为Surrogate的要求，并不代表您的健康状况或孕育能力有问题，而只是意味着您目前不符合我们针对surrogacy设定的具体资格标准，其中包括确保IVF成功所需的相关条件。
完成初始申请后，我们的Surrogate团队将与您取得联系，在我们的Surrogate Portal中继续推进申请流程。
Surrogate 门户
在我们的Surrogate团队审核您的申请、收集任何补充或澄清信息，并确认您符合所有要求之后，您将获邀加入我们的Surrogate Portal。我们的Surrogate Portal会引导您完成一系列简单的任务，让您逐步接近获得批准，并为与intended parents进行匹配做好准备。
您将被问及一系列关于您健康史和匹配偏好的深入问题，还需要填写医疗信息授权表等文件。收集到您的病史和相关记录后，我们会将这些信息安全地提供给您的intended parents所在诊所进行审核，作为您医学screening的一部分。
Surrogate 咨询
Once you complete all of your portal tasks, and both you and the surrogate team feel you are a strong candidate for surrogacy, you’ll advance to the next phase of the process, the Surrogacy Consultation. This meeting with a licensed mental health professional will be a video call to talk more in-depth about your motivations for becoming a surrogate, what your support system will look like, what your interests are and more. This video call is also a great opportunity for you to ask any questions you might still have.
